Adult Day Program

 


After careful consideration, Hôtel-Dieu Grace Healthcare will no longer be providing the Adult Day Program (ADP).  All program participants will be supported as they transition into an appropriate available program or service prior to the closure date. The closure date is anticipated to be December 2015.  This program is offered by our community partners and has been identified as a program that is best suited with these partners.

 

We understand that to many of our clients, this is more than just a program; HDGH is committed to ensuring your transition is as smooth and as easy as possible.

 

In collaboration with the CCAC all ADP participants will have an evaluation completed using the RAI CHA assessment tool.  These assessments will take place over the next few weeks and are essential to identifying each participant’s individual needs and the most appropriate community program/service.    

 

To further assist clients with this transition, an information session will be held in the Emara building on the on the following dates:

·         Monday, July 27 from 4:00 p.m. to 6:00 p.m.

·         Tuesday, July 28 from 7:00 p.m. to 9:00 p.m.

·         Wednesday, July 29 from 11:00 a.m. to 1:00 p.m.

We encourage participants and their families to attend as representatives from HDGH, the CCAC and other community partners will be on hand to discuss the next steps and program options.

 

We apologize for any inconvenience or anxiety this may have caused to our clients and their families; HDGH remains committed that all current ADP clients who continue to require services will be provided with an appropriate community program.

1. What is happening with the Day Program at HDGH?

a.       The Adult Day Program will be closing at HDGH.

 

2. When is the program closing?

a.   All program participants will be supported as they transition into an appropriate available program or service prior to the closure date. The closure date is anticipated to be December 2015.  

 

a. Hôtel-Dieu Grace Healthcare, in collaboration with the Erie St. Clair Local Health Integration Network (ESC LHIN) will be transitioning the Adult Day Program to a community based model, as it has been identified as a program better suited in the community. Moving the Adult Day Program Participants into the community allows for the enhanced reintegration of people into community programming, which is best for this type of care. HDGH, alongside the ESC LHIN, are committed to ensuring patients are receiving the appropriate care, at the appropriate location.

 

3. Who will be providing this service to the community?

 

a.  Examples of other service providers include the Alzheimer Society and VON; however it is important to know that each client requires an assessment to determine where they are best to continue their appropriate programming.

 

4. Are there any wait lists for the new program?

a.    Currently, the ESC LHIN is working with other providers to ensure all ADP patients will have a program to go to. The RAI CHA assessments will be an important piece to this, as it will identify each individuals needs and their appropriate programming.

 

5. Who determines were I will be going? Do I have a choice?

a.   Hôtel-Dieu Grace Healthcare, in collaboration with the CCAC, will have an evaluation completed using the RAI CHA assessment tool on all current ADP clients.  These assessments will take place over the next few weeks, beginning on July 13th and are essential to identifying each participant’s individual needs and the most appropriate community program/service.

 

6. When will the assessments be completed?

a.   The goal is to have these assessments completed by the end of July.

 

7. What happens if I do not want to go to another program?

a.    Ultimately it is your choice to continue to participate in another Adult Day Program. A recommendation will be made as to the appropriate Adult Day Program for you but it is your choice to decide whether you would still like to participate.  Unfortunately, HDGH will no longer be providing the program so remaining at HDGH is not an option.

 

8. Can I transfer into another program prior to the closure date?

a.  Yes, you are welcome to transfer to another program prior to the closure date.

 

9. How long will the ESC LHIN be grandfathering the $15 program fee?

a.   The fee will be grandfathered for 18 months after the client transitions. At that point the fee will be re-evaluated.

 

10. When is the information session being held? Can I bring my family so they understand the changes?

a.   Yes, we encourage you to bring your family member or caregiver with you to the information session. The information session will be held in the Emara building on the following dates to assist with your transition:

                                                               i.      Monday, July 27 from 4:00 p.m. to 6:00 p.m.

12. Will I be able to attend the new programs on the same days I currently go to ADP?

a.Yes, if you currently go on Tuesday and Thursdays for example, you will transition into a Tuesday, Thursday schedule if that is what you wish.

 

13.Where will be the location of the new Adult Day Program through VON?

a.The location is currently being identified. Once a location has been confirmed we will update this website. We can confirm that the location will be within Windsor.

 

14.The staff of the HDGH ADP program currently assists me with my taking my medication. Will the new programs?

a.Yes, the staff can assist with you with taking medications.  

 

15 .Will VON and Alzheimer’s Society come into our program to see what we do? 

a.Yes, over the next while, staff will be coming into the HDGH ADP program to see what our clients are used to. Specifically, VON will be working with our clients to develop the future ADP program at VON. 

 

16. When will I find out where I am going?

a.Assessments are currently being completed. Once all assessments are done, clients will be notified of their recommended location.

 

17. Will all of the other clients come to the same program as me? I really like this group of people… they are like a family? 

a.As indicated earlier, assessments will be completed and recommendations will be made. Each client will have a

choice as to where they would like to go.

 

18. Does the VON have a pet therapy and music program?

a.Yes, all of these pieces that our current ADP clients enjoy will be built into the new VON Program

 

19. I really enjoy playing euchre with the group – will this continue?

a.Euchre and other concentration games are perfect examples of activities that will be built into the programming at VON. In addition, the program at Alzheimer’s already has these built into its
program.

 

20.We currently go on field trips as a group? Will these still occur?

a.Yes, group outings are important for the group, so these outings will continue with the new providers

 

21.Will the HDGH ADP program be holding their annual Christmas Party?

a. Yes, the Christmas Party will be held in December. Stay tuned for more information
